OXSS Report Form

This is an example report form only, intended to guide you as to the level of detail required.  To complete a report, please go back to the Tutor Portal and open the other link, it is password protected.

Some students appear twice on the list below. If their given name is, for example, Yuxi but they mostly go by Alina, both names will appear on the list. It doesn't matter which one you use. They are both there so that it is easier to find them.

Be extra careful!

We may have some students with the same first name and last initial.  Please check that you are selecting the correct student before continuing.

If your student is not on the above list please let us (nick@oxss.co.uk and kirsty@oxss.co.uk) know and we will add them as soon as possible.

Often our students use an alternative name to the "formal" name they use on their application. Please give then name you addressed them by below - please don't give a "not known" answer - you must have called them something! This also enables us to double check that we have the right student getting the right report.

If you taught the same student for 2 or more blocks for the same subject you only need to complete 1 report.

This counts as 1 reports as if covers 1 teaching blocks.
