We would like to understand how you felt it went with each of your subject tutors.  On your timetable you will see the initials for your tutor(s), please enter these below and then answer the required question.  If you had the same tutor more than once, please note them multiple times.  If there are any additional comments that you would like to give please note these in the appropriate box.  Please note that this form is anonymous, if there was an issue during the course this should have been raised at the time it occurred so that we could effectively sort this for you.
